What you need

Waste, fraud, or abuse reports can be made to either a dedicated 24-hour voicemail phone line, fax, email, or through the mail. The report should include essential information needed to conduct a review, including the nature, scope, and time frame of the activity in question:

  • What was done or not done that was wrong?
  • By whom and in what role?
  • When and where did events occur?
  • What policy, rule or law was not followed?

Reports will remain confidential. Even though contact information of the reporting person is not required, it is recommended to allow follow up for additional information if needed to conduct an appropriate investigation. Reporters may include documentation to support the report but should not send original documents since submitted items will not be returned.

Reports will be acted on by the Office of Court Management Internal Audit staff or may be referred to departments, such as Human Resources, Legal, or Department Administrative Offices, as appropriate, for resolution and follow up. All reports will be recorded and tracked.
