What you need for Request a MATRIS user account

Individuals and vendors working on behalf of a licensed ambulance service who do not have a login to access the Massachusetts Ambulance Trip Record Information System (MATRIS) may request to be set up as a new user in the system. In order to request a user account, individuals must first complete the MATRIS Use Agreement.

How to request Request a MATRIS user account

  1. To request a user account, complete the MATRIS Use Agreement
  2. Scan the completed agreement and email it to matrissupport@state.ma.us

  2. Mail the completed agreement to:
