  • The exact location: the street address and specifics about where the issue is at that address, e.g. "123 Main St, Mytown, 01222: the West entrance from the parking lot behind the building."
  • Your list of questions
  • (Optional, but often helpful for accurately communicating the situation) Photos or a sketch of the area of concern, e.g. a close-up of a handrail on a ramp, plus a wider picture showing the entire ramp and where it leads from/to.
