What you need for Submit a plan of correction for ambulance or EFR services
Following an onsite inspection, an ambulance or EFR service may be required to submit a plan of correction to address deficiencies during the inspection. Please complete the Plan of Correction (PDF) l (DOC) form in full, including information about each vehicle, if applicable. You will also need your inspection and deficiencies report(s), to reference citations.
How to submit Submit a plan of correction for ambulance or EFR services
- Download and complete the Plan of Correction form (PDF) l (DOC)
- Email the completed form to oems.ambulance@mass.gov or directly to the assigned Inspector
- Subject Line: Plan of Correction – “Service Name” – “Service Number”
