Submit a plan of correction for ambulance or EFR services

If you have reported deficiencies, you can submit details of corrective action you are taking.

What you need

Following an onsite inspection, an ambulance or EFR service may be required to submit a plan of correction to address deficiencies during the inspection. Please complete the Plan of Correction (PDF) l (DOC) form in full, including information about each vehicle, if applicable. You will also need your inspection and deficiencies report(s), to reference citations. 

How to submit

  1. Download and complete the Plan of Correction form (PDF) l (DOC)
  2. Email the completed form to oems.ambulance@mass.gov or directly to the assigned Inspector
    1. Subject Line: Plan of Correction – “Service Name” – “Service Number”
