What you need
Employers may qualify for WOTC when they hire employees from certain targeted groups. When an employment offer is made, both the employer and job applicant need to:
- Complete and sign IRS Form 8850, Pre-Screening Notice and Certification Request for the Work Opportunity Tax Credit AND
- Complete and sign ETA Form 9062, Conditional Certification Form (PDF) if it was provided to the jobseeker by a MassHire Career Center or participating employment agency, OR
- Complete and sign ETA Form 9061, Individual Characteristics Form (PDF)
