  1. Before You Apply

    Contact the appropriate MassDEP Regional Office to schedule a pre-application conference. This will help you prepare a better application, saving you time and likely resulting in faster review. If you apply without setting up a meeting, MassDEP may require one anyway before it can determine whether your application is administratively complete.

  2. Review the Instructions

    Guidance on how to prepare your application and file it online, including a completeness checklist.

  3. Complete the Application Form

    You may fill out and save the Microsoft Word form on your computer. The PDF version has to be printed out and completed by hand.

    Important Note: Even when applying online, you will still need to complete a conventional Application Form and upload it as an attachment during the online filing process.

  4. Submit Your Application & Fee Payment

    1. File online via the EEA ePlace Portal.

      Note: If you are a first-time user, you will need to create an account first.
       
    2. Pay your fee online with a credit card and submit your application, or you may mail your fee payment to: 

      Department of Environmental Protection
      P. O. Box 4062
      Boston, MA 02211
       
    3. Submit a copy of your completed Form SW 32, 33, 35 to the local Board of Health.
