DDS Safety Plan Guidelines and Templates

DDS providers must maintain site-specific Safety Plans (with limited exceptions) that are accessible on-site and reflect individuals’ needs, staffing supports, and evacuation strategies, consistent with 115 CMR 7.06(3) (Safety Plans).

Implementation Timeline

Implementation of the revised Safety Plan begins April 1, 2026, and will occur over a two-year transition period. When developing new Safety Plans or updating existing plans, providers must use the revised standardized Safety Plan format required for the applicable service. 

Safety Plans must be developed or updated:

  • When new site opens or a site relocates
  • Within two years, if no changes affect safe evacuation
  • When Safety Plans are no longer effective or proposed changes occur
  • When the provider agency operating a site changes

Required Appendices and Tools

Appendix D: Individuals’ Abilities and Evacuation Needs Chart (Required)  Use this chart to document each individual’s evacuation ability, staff support required, supports provided, and any devices used (e.g., wheelchair, walker, Hoyer lift, bed shaker).

Appendix E: Site Floor Plan (Required)  Attach a floor plan for each level accessed by individuals, clearly marking all means of egress using the standardized egress codes (e.g., interior stairs, door to grade, accessible ramp).

Appendix F: Safety Plan Regulatory Applicability by DDS Service Type (Reference)  Quick chart showing which Safety Plan components are required (or not applicable) by DDS service type, including key site-based requirements such as egress/accessibility and fire drill expectations.
