Duplicate license/change of address/change of name requests: operators of drinking water supply facilities

Information on requesting a duplicate license, change of address or change of name requests.
  • All applicants must complete an application on the Board’s eLIPSE platform. 

  • When you create your MyMassGov account to access eLIPSE the first time, eLIPSE will ask you to enter information to help it locate your license and link it to your MyMassGov account.  

    • Please read the Linking Your License guide before creating your account to ensure it is linked correctly.

Important Points to be aware of before you start:

Legal name change requires uploading a copy of the court document or marriage certificate etc. and surrender of the old license to the Board office. If unable to upload the documents, mail in or fax to 617-727-1627 and attach the application ID reference number to the document.
