Guidance for Employee Self-Service and E-Forms

Manage employee information and enroll in certain programs managed by the Employee Service Center (ESC).

Employees can take care of many tasks through Employee Self-Service (ESS)—the online portal where employees can enter time, view pay statements, access tax information, and more. Some updates require employees to use electronic forms (E-Forms), and in certain cases, employees may need to provide additional details. Enrollment forms for programs managed by the Employee Service Center are also outlined on this page.

Employee Information that can be updated in Employee Self-Service

The items below can be easily updated by employees through Employee Self-Service.  Instructions for each change have been linked below.  (Tip: Some updates can also be made using an E-Form. However, we recommend starting with Employee Self-Service whenever possible as it assures the quickest results.)

Employee Information that can be updated using Ethnicity Change Form

In addition to Employee Self-Service, you may complete the Ethnicity Change Form to update the following:

Public Service Loan Forgiveness (PSLF)

For employees applying for Public Service Loan Forgiveness (PSLF), the Employee Service Center (ESC) provides employment verification on behalf of the Commonwealth. The ESC’s role is limited to this verification step. Completion of all federal PSLF form requirements and submission to the U.S. Department of Education remain the responsibility of the employee.

There are two options for how to submit your Public Service Loan Forgiveness (PSLF) Form.

Option 1: Submit your Form using PSLF Help Tool 

Option 2: Submit your PSLF Form manually

If you prefer not to use the PSLF Help Tool, you can download and print a blank PDF of the form, complete all sections, sign it, and have your employer(s) sign it.  Note: Completing your PSLF form in this manner may add additional time to the processing of your form.

Complete the Public Service Loan Forgiveness (PSLF) Form. Review the Public Service Loan Forgiveness Program page for information on eligibility and usage.

  • PSLF Form Instructions:
    • Complete Page 1 only of the PSLF form
    • Include the last four digits of your Social Security number on the form
    • Email masshremployeeservicecenter@mass.gov with a copy of your PSLF form as a PDF and include your Employee ID number
    • Unless otherwise notified, the Employee Service Center will sign and complete the form within 10 business days.
