What is the ADH Program?
ADH is a community-based, non-residential service that provides eligible MassHealth members with skilled services and assistance with Activities of Daily Living (ADL) in a structured setting.
ADLs are fundamental personal care tasks performed daily as part of an individual’s routine self-care, which include, but are not limited to:
- Eating
- Toileting
- Transferring
- Mobility/Ambulation
ADH facilities are required to be open Monday-Friday, for a minimum of 8 hours each day. While there may be slight variation between programs, typical services offered include:
- Social, recreational, educational, and therapeutic activities
- Case management
- Individual and/or group counseling
- Medication management
- Health monitoring
- Nutrition services
- Skilled nursing
- Restorative therapies (physical, occupational, speech)
ADH programs are licensed by the Department of Public Health as health care facilities.

Eligibility Criteria
Applicants must be found clinically and financially eligible. Below are the required criteria for all ADH applicants.
Clinical Eligibility
All ADH applicants must meet the following requirements:
- Be a Massachusetts resident
- Be 18 years of age or older
- Have a physical, cognitive, or behavioral health need
- Require assistance with at least one activity of daily living (ADL) or require skilled nursing services
- Have a primary care provider (PCP) deem that ADH services are medically necessary
Financial Eligibility
Applicants must also meet the financial requirements of MassHealth.
In addition to MassHealth Standard, applicants may also gain eligibility through CommonHealth or be authorized for ADH services through MassHealth’s One Care or Senior Care Options (SCO) programs.
Admission Process
Interested in attending an ADH program? The process is straightforward:
- Find a program that fits your needs. Use MassHealth’s interactive map to filter for location, language, and specialized needs Adult Day Health (ADH) Programs map.
- Contact the program to arrange for a visit and/or express interest in attendance.
- Schedule a visit with your PCP to complete the required Adult Day Health Primary Care Provider (PCP) Order Form. In order to attend ADH, you must complete a physical exam within the past 12 months.
- A nurse will call to schedule an assessment to be completed in your home that will be used to determine ADH eligibility.
- You will receive a letter of determination in the mail to let you know if you are eligible to receive ADH services through MassHealth.
- Once eligibility has been confirmed, the program will arrange a start date and transportation, if needed, and you will be ready to start attending the program.
