Mandatory Report of Legal Blindness
We encourage optometrists and ophthalmologists to submit the Mandatory Report of Legal Blindness via our online portal for quick and efficient registration.

What Happens Next?
Once you complete the Mandatory Report of Legal Blindness form and click "Next," you will be directed to a webpage with a five-digit Mandatory Report Form ID (confirmation number). Be sure to note this number for reference if you need to contact our Blindness Registry about the patient’s registration status.
After submitting the report, an email will be sent to the provided address confirming receipt. Our Blindness Registry will review the report for eligibility within one to three business days.
- If the report is certified, our Blindness Registry will mail a letter of determination to both the patient and the eye care provider. The patient will then be registered and assigned a counselor. The assigned counselor will contact the newly registered individual within two weeks to schedule an appointment and explain the benefits, programs, and services available.
- If the report is rejected due to missing information, the Blindness Registry team will contact the provider by email or phone for further details.
- If the report is rejected because the patient’s vision does not meet legal blindness criteria, the Blindness Registry team will mail a letter to both the patient and eye care provider.

Eye Care Provider Access
If you are in need of an eye care provider, please contact your insurance provider to inquire about covered eye care services and find a provider within your network. Your insurance provider can assist you in finding a suitable professional for eye exams, prescriptions, and other eye care needs. Be sure to confirm any details regarding coverage, including co-pays, deductibles, and network restrictions.
