Online Reporting for Eye Care Providers

All eye care providers must report within thirty (30) days all cases of legal blindness to the Massachusetts Commission for the Blind pursuant to Mass. Gen. Laws ch. 6, § 136.

Mandatory Report of Legal Blindness

We encourage optometrists and ophthalmologists to submit the Mandatory Report of Legal Blindness via our online portal for quick and efficient registration.

If you are in need of an eye care provider, please contact your insurance provider to inquire about covered eye care services and find a provider within your network. Your insurance provider can assist you in finding a suitable professional for eye exams, prescriptions, and other eye care needs. Be sure to confirm any details regarding coverage, including co-pays, deductibles, and network restrictions.
