With the release of the Disability Component in PERAC's PROSPER application, Medical Vendors have a secure and convenient portal for processing disability cases and submitting invoices. The tutorials below provide additional guidance.

Medical Vendor Requirements

In order to request approval to access the PROSPER application, Medical Vendors must submit the following to PERAC: 

• PROSPER Business Associate Agreement
  (emailed directly to vendors by PERAC)
• PROSPER Medical Vendor Account Approval Form 
• Updated Curriculum Vitae
• Massachusetts Substitute W-9 Form 
• Commonwealth Terms and Conditions
