3rd Amended and Restated ACPP Contracts

This page contains EOHHS’s contracts with its Accountable Care Partnership Plans (ACPPs).

The “base” ACPP Contract and several appendices are the same across all ACPPs. There are certain appendices (and other Contract documents, such as the Contract signature page) that are plan-specific. These documents are all below. Please note that the 3rd Amended and Restated (A&R) ACPP Contract supersedes previous versions and incorporates all applicable provisions from those previous versions.
