BORIM is providing a comprehensive set of resources for the Training Facilities to use the Electronic Licensing Excellence (eLX) Facility module, and to reference the downloadable forms that are part of the application process.

Downloadable Forms for Applicants

These pre-filled forms are available to the applicant in their user account during the application process and after submittal to the training facility.  Completed forms may be uploaded by the applicant, returned to BORIM or the Training Facility, or in the case of the Supervisory Evaluation, the evaluator can upload the file with a secure link provided by the applicant.  Instructions appear on each form.
