All complaints must contain an allegation that a CSL holder has violated a provision of the Massachusetts State Building Code 780 CMR and a letter or report from the building inspector, an engineer, or architect citing the code violations.

To file by mail, complete the attached form in its entirety and mail the documents to:

Applicants will be notified in writing if a hearing is scheduled based on the merits of the complaint.
