Submitting your surcharge fee

If you are a service provider and would like to do business in Massachusetts, you must submit your surcharge fee. You must also review and fill out all necessary forms and procedures, depending on your service.

All service providers who would like to do business in Massachusetts must submit your monthly surcharge. To submit your surcharge:
