Submit your 91A Annual Statement of Earned Income Form online

Disability retirees now have the ability to submit their 91A Annual Statement of Earned Income information completely online. You will receive your 91A Form in February along with instructions on how to complete the form online.

Create a PDF with your phone!

All uploads to the online form must be in PDF format. Learn how to create PDFs with your phone if you do not have access to your documents in PDF or do not have a scanner. If you need additional assistance, contact the 91A HelpDesk by email or by calling 617-591-8991.
