Submit your 91A Annual Statement of Earned Income Form online

Disability retirees now have the ability to submit their 91A Annual Statement of Earned Income information completely online. You will receive your 91A Form in February along with instructions on how to complete the form online.

Why do I have to file a 91A form?  

All disability retirees must file an Annual Statement of Earned Income (Form 91A). Failure to comply could result in the termination of your retirement benefits.
