Bridging School to Community: A Family Roadmap

Please join us for a free Fall Transition Event on Thursday, October 23, at Perkins.

This morning event is designed for families and professionals supporting young adults with complex support needs who will transition to adult community services after high school graduation. You’ll hear from speakers from different agencies who will help you understand available supports and share what daily life and services look like in community-based programs.

The day will include an interactive panel discussion with community agency representatives, including the Department of Developmental Services, Massachusetts Commission for the Blind, as well as community-based day program representatives.  You’ll gain a clear understanding of the application process, how eligibility is determined and how both you and your young adult can play an active role in shaping the transition journey.
