Information Session on Proposed Standard Prior Authorization Forms

Overview

The Division of Insurance has scheduled an information session to consider comments about proposed standard prior authorization forms as submitted by the Mass Collaborative, an organization that is a collaborative of Massachusetts providers and payers. 

The first two proposed forms - developed in response to Section 25 of Chapter 197 of the Acts of 2024 - are for the following types of services:

           Post-Acute Care 

           Home Health Care

The third proposed form amends a previously approved form in response to Chapter 388 of the Acts of 2024:

           Applied Behavioral Analysis (amended to include reference to Down Syndrome).         

Carriers are required to use standard prior authorization forms, according to M.G.L. c.176O, §25, as they are approved by DOI, when considering requests for services. The DOI developed a form as the standard for carriers and providers to use when requesting chemotherapy services within the utilization review process used by an insured health plans. The scheduled March 14, 2022 session is intended to hear comments about the adoption of this form.
