Overview   of September 19, 2018 MOLST Subcommittee Meeting

The Medical Orders of Life Sustaining Treatment (MOLST) Subcommittee advises on updates to the MOLST form. We work with the Palliative Care and Quality of Life Interdisciplinary Advisory Council and the Department of Public Health (DPH).

Agenda

Our Mission: To create a team-based, conversation driven MOLST process and document, which is responsive to an adult’s changing health needs and choices all through the serious illness journey.

  1. Welcome & Introductions
  2. Building an improved MOLST structure. Slides and talking points:
    • What do we know?
      • Identify foundational elements; MA law & state regulations; what’s working well
    • What have we learned?
      • Identify implementation challenges, barriers & gaps to know & honor choices
      • Share your program ideas for quality conversations & team-based approaches
    • What should we include?
      • Widen the lens to include all adults, all care providers; POLST Paradigm requirements
  3. Basics First: 10 Questions to identify a common MOLST language and shared process
  4. Look Ahead: Changes to MA MOLST document
  5. Next Steps: Group discussion
