Public Records Request Form

This form is provided by the Massachusetts Department of Family and Medical Leave (DFML) for requesting public DFML records only.

  • Do not use this form to request records from your own personal Paid Family and Medical Leave (PFML) case file. Case files are considered confidential and are not a general public record. There is a separate form for PFML case file requests.
  • Do not use this form to request records from organizations, departments, or programs other than DFML. If you wish to request records from a different entity, please visit that entity’s webpage for information on how to request records.
