Background information:

To inform the implementation timelines and guidance around 115 CMR.50 Standards to Promote Dignity the Department of Developmental Services (DDS) is seeking information pertaining to providers' current restraint, behavior management, and behavior plans.

Please fill out the survey below to help us determine the number and contents of current Level 1 and 2 behavior plans, positive behavior support plans (PBSP), and written behavior guidelines not contained in a Level 1 or 2 plan or PBSP.

If a provider operates in multiple regions, please complete the survey separately for each region.
