Listed voucher reimbursement rates go into effect for all services submitted on or after July 1st, 2023 for all contracted partners.

Voucher Request Forms

Animals in the Community

Veterinary Providers and Municipal Partners requesting vouchers for individuals should use the Owned Animal Request Form. 

Animals in ACO Care

Municipal Partners requesting vouchers for animals in ACO Care can use the ACO Request Form. 

Vaccine Vouchers

To request vaccine vouchers for animals in ACO ( that have already been spayed/neutered) submit a Vaccine Only Request Form.
