What you need for Add Place of Business or Vehicle
Vehicles
Before you put any new vehicles into operation, each vehicle must be certified. You must submit a weight verification form and pay the $200 fee for each vehicle that you are adding to the ambulance service.
Place of Business
Before you add any additional locations, each location must be approved and appear on your Service license. If the location intends to garage ambulances or store medications, you must have an Ambulance Service Controlled Substance Registration (MCSR) through the Drug Control Program (DCP). For the purpose of applying for your new location with OEMS, there must be a pending MCSR application on file with DCP prior to submitting your application with OEMS.

How to apply Add Place of Business or Vehicle
To add a new vehicle or location, complete the Update or Add Place of Business or Vehicle Application, submit payment (if required), then submit. Please note, submission comes after payment.
- Log onto the Health Professions Licensing Portal
- Log in to your account and select “Manage My Licenses and Applications” and then “Show Details” on your Ambulance or Emergency First Responder Service license.
- Select the “Update or Add Place of Business or Vehicle” application
- Complete the application, submit payment (if required), then submit. Please note, submission comes after payment.
- If you service cannot submit an online payment, please complete and mail the Ambulance Regulation Program Mail Fee Submission Form (PDF) l (DOCX) with your check or money order.
- If you submit payment through the mail, once received and entered, OEMS staff will officially submit the application on your behalf.

If a licensed Ambulance/EFR Service wishes to use a certified Ambulance/EFR vehicle from another service for a temporary amount of time, both the service providing and the service receiving the vehicle must complete the Multi-Service Vehicle Use Disclosure Form (PDF) l (DOCX) disclosure form.
A representative from either service may submit the form to oems.ambulance@mass.gov.
