What you need for Apply for a Plumbing Business License
To complete a business application, the following information must be submitted with your online application through the ePLACE portal:
- Organizing Documents: A signed, stamped copy of your businesses' organizing documents as filed with the Massachusetts Secretary of State. Based on the corporate structure of your business, please provide the Board with the following documents:
- Corporation or Professional Corporation (PC): Signed, stamped copy of the Articles of Organization filed with the Massachusetts Secretary of State. Please note that this document must state the the purpose of the corporation is plumbing. In addition, the master plumber of record must be listed as an officer of the corporation.
- Limited Liability Corporation (LLC): Signed, stamped copy of the Certificate of Organization filed with the Massachusetts Secretary of State. Please note that this document must state that the primary purpose of the LLC is plumbing. In addition, the master plumber of record must be listed as a manager of the LLC.
- Limited Partnership (LP) or Limited Liability Partnership (LLP): Signed, stamped copy of the Partnership Certificate filed with the Massachusetts Secretary of State. All listed partners must be licensed master plumbers.
- General Partnerships: A written agreement signed by all partners and notarized, creating the general partnership. All listed partners must be licensed master plumbers.
- Criminal Offender Record Information Acknowledgement (CORI) Form: All master plumbers listed as officers, managers, or partners in the business (as appropriate, based on the corporate structure of the business) must pass a CORI check prior to approval of the license. Please download and complete the CORI Form for each such individual associated with the business. Please ensure that the second page of the form is notarized.
Note for Out of State Businesses: All out of state applicants MUST provide a signed and stamped copy of the Foreign Corporation Certificate (for corporations) or Foreign LLC Certificate (for LLC's) or such other document(s) as required by the Massachusetts Secretary of State.
