What you need   for Apply for a replacement Section 5 plate

If you need a temporary plate:

  • Complete the application
  • Submit the application with the Master Registration and a $10.00 fee 
  • A Temporary Plate can be issued, if needed

Once you receive the replacement plate, you need to return the temporary plate either in person or by mail to:

If you do not require a temporary plate, please submit your application, Master Registration, and $10.00 by mail to the address above.
