What you need   for Become a MassDOT approved fabricator

Fabricators wishing to be approved by the Department must submit the following:

  • Completed prospective approved fabricator application
  • A description of the facility including:
    • history
    • capacity
    • equipment
  • The Fabricator's Quality Systems manual
  • A Table of Organization
  • Representative samples of Welding Procedure Specifications and Welding Procedure Qualification Test Records
  • Representative samples of Welder and Welder Operator Qualification Test Records
  • Resumes of supervisory personnel and the resumes of all personnel involved with quality assurance, quality control & testing
  • A copy of the American Institute of Steel Construction Quality Program Certificate

Upon receipt

The completed application and attached materials will be reviewed and a shop audit will be conducted. The applicant may withdraw their product application from the evaluation process at any time by submitting a written request to the above address.
