  • Name of your employer and its labor relations representative/attorney, and their street and email addresses
  • Name of your union and its representative/attorney, their street address, and email addresses
  • The remedy you are seeking

  2. Electronically sign the declaration and the certificate of service
  3. Follow the instructions to electronically submit a copy to the employer representative or the union representative
