What you need

If you would like to donate to the Conservation Trust and Urban Parks Trust Fund, you will need:

  • A completed Contribution Form
  • The specific park or program you would like your donation to support (if you don't have a preference, your contribution will go to wherever the need is greatest)
  • A check made payable to the Commonwealth of Massachusetts

How to file

You will need to mail your completed contribution form and your donation check to:
