What you need   for File an Appeal with DTA

A written or verbal telephone appeal request.  Appeal requests are usually found on most DTA notices telling you about a change in your benefits.

A written or verbal telephone appeal can be requested:

  1. Using a Department of Transitional Assistance Appeal Form (sent to you with notices); 
  2. Written on paper; or
  3. By calling the DOH and leaving a detailed message as outlined below

Your appeal request must contain:

  1. Your name
  2. Your mailing address (must be a street address or P.O. box if you do not have one)
  3. A statement of what you are appealing
  4. Your DTA Agency ID number (This can be found on any notice DTA has sent you)
  5. Best phone number to reach you at

You can visit a local DTA office to use our phones, copiers, or kiosks to file an appeal.

Next steps

  1. Once Division of Hearings (DOH) receives your appeal request, a telephonic hearing will be scheduled.
  2. You will receive notice of the date, time, and location of the hearing. It will be mailed to you 15 days before the hearing.
  3. DTA will call you for the hearing. Most hearings take from 30 minutes to one hour.
  4. After your hearing, the Hearing Officer will make a decision and mail it to you within 30 days.
