Report a name change to DUA

If you have legally changed your name, you must report that change to the Department of Unemployment Assistance (DUA).
Once you have changed your name with the Social Security Administration and received your new Social Security card, you must report your name change to DUA as soon as possible.

What you need

Before you can report your name change to the Department of Unemployment Assistance (DUA), you must first change your name with the Social Security Administration.

After you receive your new Social Security card, you should report your name change to DUA. You will need the following:

  • A copy of your new Social Security card
  • Legal documentation of your name change (i.e. court document, marriage license)

How to report

To report your name change online:

  1. Log in to your Unemployment Services for Workers account
  2. Select Additional services
  3. Under Name and Addresses, select Manage name and address
  4. Select your name
  5. Select Change this name
  6. Follow the system prompts to report the name change
