The Details

What you need

After you change your name with the Social Security Administration, you can update your information with DUA. 

You will need:

  • A copy of your new Social Security card
  • Legal documentation of your name change (for example a court document or marriage license)

How to report

  • Log in to your Unemployment Services for Workers account
  • Select Additional services
  • Under Name and Addresses, select Manage name and address
  • Select your name
  • Select Change this name
  • Follow the prompts to complete the update
