Instructions for certified EMT-Basics, Advanced EMTs, and Paramedics to meet the reporting obligations of EMS personnel under 105 CMR 170.937. Please carefully read the instructions below.

What you need

Reporting Obligations of EMS Personnel under 105 CMR 170.937:

  • For certified EMT-Basics, Advanced EMTs, or Paramedics, the EMS System regulations require you to report the following incidents within five calendar days, to your employing ambulance or EFR service, as well as to the DPH/OEMS by completing the online EMT Self-Reporting form:
    • Your conviction of a misdemeanor or felony in Massachusetts or any other state, the United States, or a foreign country (including a guilty plea, nolo contendre or admission to sufficient facts, even if the plea or admission results in a continuation without finding), other than a minor traffic violation for which less than $1000 was assessed. The following traffic violations are not minor and must be reported: conviction for driving under the influence, reckless driving, driving to endanger, and motor vehicle homicide; 
    • Your loss or suspension of your driver’s license; 
    • Disciplinary action taken against you by another governmental licensing jurisdiction (state, United States or foreign) or the NREMT, against an EMT or other health care certification or license you hold; or 
    • Suspension or revocation of your authorization to practice by your service’s affiliate hospital medical director.

You may also use this form to submit follow-up documentation for a previous online EMT Self-Reporting form submission.

  • If you need to submit follow-up documentation for a previous submission, please ensure you have your original EMT Self-Reporting form submission confirmation number (example: EMTSRF12345), received via email.
  • With the original EMT Self-Reporting form submission confirmation number, complete another online EMT Self-Reporting form, select the “follow-up documentation” option, enter the submission confirmation number, upload the relevant documentation, and submit.

  1. Provide a detailed explanation, include your certification number, and  any relevant documentation.
  2. Fax your completed form to (617) 753-7320.

If you are unable to submit an EMT Self-Reporting form online you may:

  1. Provide a detailed explanation, include your certification number, and any relevant documentation.
  2. Email your completed form to oems.recert@mass.gov.
  3. Do not email sensitive information such as social security numbers, dates of birth, patient care reports, etc.
