If you need proof of your income from unemployment benefits, learn how to request it from the Department of Unemployment Assistance (DUA).

What you need   for Request a letter verifying your unemployment benefits income

If you’re applying for a mortgage, heat assistance, or housing, you may be asked to provide proof of your income from unemployment benefits.  

In some cases, simply printing the records from your UI Online account may be enough. But if you’re asked for a formal letter or records from DUA, you must fill out a Release of Information for Claimant form and submit it to DUA. 

Here’s the information you need: 

  • Time period covered  
  • Type of document (for example – unemployment case file with dates of claim, Hearing or Board of Review materials including files and tape(s) with specific dates and any docket numbers, etc.) 
  • Full name 
  • Claimant ID number or last 4 digits of Social Security number 
  • Date of birth 
  • Address

To request a letter by email, download the Release of Information for Claimant form and fill it out. Email the completed form to: duapublicrecordsrequest@detma.org
