What you need

Municipal Police Departments may use this form to request DCJIS review of a by-law or ordinance seeking National Fingerprint-Based Criminal History Check for municipal license applicants pursuant to M.G.L. c. 6, § 172B ½. Please see the Department of Criminal Justice Information Services Policies and Procedures for Civil Fingerprinting for Municipal Licensing Applicants for a detailed description of the approval process. Fingerprint checks for municipal license applicants cannot be completed until all steps have been completed. Please upload all relevant documentation, including your municipality’s by-law and fingerprinting policy.
