What you need
Court Order for Criminal Offender Record Information (CORI) - An attorney seeking CORI in association with a pending criminal or civil court matter must submit a signed court order.
To submit a motion, you will need:
Signed court order that:
- Includes the name, date of birth, and last six digits of the social security number for each subject for whom CORI is to be provided.
- Specifies the level of CORI access granted by the Court for each subject
- The level of CORI access for court orders will either be Required 1 which will include conviction information only or Required 2 which will include all available CORI information including both conviction and non-conviction information.
- $25 fee per subject
If you are court appointed counsel, you may attach a copy of the Notice of Appointment of Counsel (NAC) form or your NAC number and the fee will be waived.
*Requests for CORI that are not made pursuant to an allowed court order must be submitted through the iCORI system.
Motion for CJIS Audit Records - Attorneys seeking access to Criminal Justice Information System (CJIS) transaction records in association with a pending criminal matter must file a motion pursuant to Rule 17 of the Mass. R. Crim. P., Commonwealth v. Dwyer, 448 Mass. 122 (2006), and Commonwealth v. Lampron, 441 Mass. 265 (2004). Motions for CJIS audit records must be submitted to DCJIS a minimum of 2 weeks prior to scheduled motion hearing date.
To submit a motion, you will need:
- Copy of the motion
- Police report(s) that serve as the basis for the record request
- Date of the motion hearing
Name of the Assistant District Attorney handling the motion for the Commonwealth
*Please note that motions requesting CJIS audit records pursuant to Rule 14 of the Mass. R. Crim. P. will not be reviewed or processed.
*Please note that DCJIS requires a minimum of 6 weeks to produce CJIS audits records as they must be retrieved, reviewed page-by-page, and redacted in accordance with state and federal statutes and regulations and the FBI CJIS Security Policy
Court Request for Firearms Records - Attorneys seeking firearms licensing records and/or transactions records in association with a pending criminal matter may submit a request to DCJIS. Pursuant to M.G.L. c. 66, § 10B, requests from defense counsel and/or plaintiff’s counsel must be accompanied by a signed court order issued pursuant to an appropriate motion. If you are a government attorney seeking a firearms licensing history, you may submit a request to DCJIS.
- To submit a record request, you will need:
- Name/date of birth for the subject of the firearms licensing history
- Docket number of underlying court matter
- Court where matter is pending
- Criminal charges
Date of trial (DCJIS will only process firearms licensing record requests in association with matters scheduled for trial 6 weeks of the request date. All other requests for firearms licensing records should be submitted to the investigating police department)
If you are a defense attorney or plaintiff’s attorney seeking firearms records, you must submit a signed court order.

- LTC/FID number of subject (if applicable)
- Firearm serial number (if applicable)
- Docket number of underlying court matter
- Court where matter is pending
