How to submit
When filing the hotline MOR, you will be instructed to print a copy of the MOR incident report page for your records; you must click “print this page” prior to clicking “submit incident report”. Keep a copy of the printed MOR at the MAP registered site where the occurrence took place

When you file an online Hotline MOR, an Incident report MOR # is assigned and appears in the top left corner of the report. Please refer to this MOR Incident report # in any communication with DPH related to this incident.

Hotline medication occurrence information
- A hotline medication occurrence is any medication occurrence that is followed by medical intervention, illness, injury, or death. View MAP Policy 17-2 Hotline Event Medication Occurrences
- If a medication occurrence meets criteria as a hotline medication occurrence, you must submit a Hotline Medication Occurrence Report to the Department of Public Health (DPH) within 24 hours of discovery of the occurrence
- Occurrence Reporting is required by regulation at 105CMR 700.003(E)(1)(f)
- Consultant Contact is required by regulation 105CMR 700.003(E)(1)(g)
The submission of the Hotline MOR form will satisfy DPH's requirement for notification to DPH within 24 hours of discovery of the occurrence.
