  2. Complete the Reporting Forms

    You may fill out and save Microsoft Word forms and Excel spreadsheets on your computer. PDF versions have to be printed out and completed by hand.

    Please Note: Revised Waste Ban Inspection Load Observation and Comparative Analysis Sheets are available for use as of September 2018. Inspectors must use these forms for all inspections beginning January 1, 2019.

    Submit your completed and signed Inspection Report (and Corrective Action Plan, if necessary) to:

    MassDEP Regional Office
    Attention: Solid Waste Management

    Send one copy of the relevant form(s) to:

    Local Board of Health

    MassDEP Regional Offices by Community
