What you need for Update your physician assistant license information or request a duplicate license
According to 263 CMR 3.03 (4), you must notify the Board in writing of any of the following events within thirty (30) days of their occurrence:
- Change of address
- Change of identity of your employer or employment status
- Any change in the identity or address of your registered supervising physician
- Your permanent departure from Massachusetts
It is important to keep your information updated. Your address on record is the Board mails your license and any correspondence. If you do not notify the Board of your new address, you may not receive a license renewal application and could miss the expiration of your license.
How to update Update your physician assistant license information or request a duplicate license
You may update your information on the Mass Department of Public Health Online Licensing site.
To request a duplicate license, you must mail in a paper form, available under 'by mail' below.
Mail your completed and signed Change of Address or Name and/or Request for Duplicate License (DOC) to:
