WW 06, 12: Chapter 91 Simplified License & License Renewal

A Chapter 91 project licensed using the simplified license option has a limited term of generally 10-15 years. For the authorization to continue, the licensee or his/her successor must request renewal of the simplified license.

Simplified procedures have been created for owners of small residential docks, piers, seawalls and bulkheads to make the Chapter 91 license application process easier and less expensive. Residential waterfront property owners should be able to complete and file the Chapter 91 license application without the need for professional assistance.

Anyone who obtains a Waterways simplified license must file an application to extend the term of the license prior to the its expiration. This includes anyone who obtained an Interim Approval under the amnesty provisions in 310 CMR 9.28.

  2. Complete the Application Form

    You may fill out and save the Microsoft Word form on your computer.

  3. Obtain a Transmittal Form and Number

    A unique Transmittal Number is needed for each application you submit to MassDEP.

  4. Look up the Permit Application Fee

    Fees for WW 06 and WW 12 are found on page 9 of the fee schedule below. 

  5. Mail your Application and Fee Payment

    Submit your application fee payment and a copy of your Transmittal Form to:

    If your project is water-dependent and is located in MassDEP's Southeast or Western Regions, send your completed application and transmittal form to the appropriate regional office (see listing below). 

    If your project is water-dependent and is located in MassDEP's Central or Northeast Regions (including Boston), send your completed application and transmittal form to:

    All applications for non-water-dependent projects should be sent to the Boston office.
