- Download and fill out Form 112 – Appeal to the Reviewing Board
- If you were hurt on or after Nov. 1, 1986, you must pay a filing fee equal to 30% of the state average weekly wage at the time of the appeal
- As of October 1, 2019, the appeal fee is $429.49
- Make the check out to the Massachusetts Industrial Special Fund
- Mail the form and fee to:

- If you can’t pay the filing fee, you may request a waiver by filing Form 112A – Affidavit in Support of request for Waiver of Filing Fee under § 11C
