Need to update your Certificate of Eligibility? This Amendment Form will assist you in gathering the information needed for your request. Before submitting it through the DCMS system.

Granting Amendments is discretionary.  Contractor must satisfy all requirements for an Amendment to be approved and granted.

INSTRUCTIONS: Once you have gathered the information needed for your Amendment Application.  Log on to your DCMS Account and Request an Amendment to your company’s Certificate of Eligibility.  You will then be prompted to submit an Amendment Request Application.
