Community Health Equity Survey Mini-grantees

DPH provided flexible mini-grants for community-based organizations to conduct outreach for the Community Health Equity Survey. Grant recipients used the funds to support community members, particularly those who experience health inequities and are traditionally left out of public health data, in completing the survey.
