Document preparation

The Department of Industrial Accidents (DIA) uses high-volume scanners to import all paper documents into its document management system (DMS). The scanners handle large amounts of paper, but the documents themselves must be prepared in a specific way in order to be scanned and stored in the proper electronic file. Document preparation is a very important part of the process!

First and foremost, documents must be presented to the DIA (either at a proceeding or via U.S. mail) without any fasteners such as paperclips, staples, binders, etc. A large binder clip or rubber band that holds an entire filing together is acceptable. Second, each document (be it 1 page or 20 pages) must be accompanied by a bar-coded cover sheet. 

The bar-coded cover sheet identifies the document, who is submitting it, the relevant dates, and other information unique to the case. The module that allows parties to create bar-code sheets is available through secured online user accounts via the DIA's website. It is required that all attorneys and insurers obtain user accounts to gain access to this module. The user accounts are provided free of charge and only require the recipient to have access to the Internet and a valid email account. To obtain an account see Apply For A DIA Online Account .

All documents submitted at conciliation, conference, or mailed to the DIA must be prepared in this manner. Initial form filings (i.e. Form 101 - First Report of Injury or Form 110 - Employee Claim) need not be accompanied by a cover sheet as the DIA will have that responsibility.

Documents submitted to the DIA electronically - via e-mail and other means - are imported into the DMS but require no additional preparation.

Bar-coded coversheets

The bar-coded cover sheets identify all paper documents that are submitted to the Department of Industrial Accidents (DIA). You need an online account to create the coversheets.

The bar-code generation is a single cover sheet that takes only seconds to create. The user needs to select the correct document type from a pull-down list and enter between 2 and 5 pieces of information about the document. For example, if you have a conference packet with 25 documents, creating cover sheets for all 25 should take about 10 to 15 minutes.

Remember that paper documents submitted to the DIA must have a cover sheet and must be free of any fasteners (no paperclips, staples, binder clips, etc. are allowed). A packet of documents held together by a large rubber band or single binder clip is ok.
