Your search criteria should include:
- Office of Emergency Medical Services for “License Board”
- Ambulance Service License or Emergency First Responder License for “License Type”
- For a full list of all services, leave “Organization Name” blank
- You may export the search results into s CSV or Excel file.
A “License Status” of “Current” indicates the service is operational, regardless of the indicated expiration date. A “License Status” of Provisional is distinct from a standard ambulance service or EFR license. The Department of Public Health frequently inspects the service during its up to six-month provisional licensure period, to provide more oversight support and monitor compliance with all regulatory standards as well as its approved Plan of Correction.
