Foreign Insurance Company Merger Instructions

A foreign insurance company with authority to transact insurance business in Massachusetts that merges must submit the NAIC UCAA Foreign Corporate Amendment Application.

Application must include the following:

1. A certified copy of Amended Articles of Incorporation of the surviving entity
2. A copy of the decision(s) approving the merger from the insurers domiciliary state insurance departments
3. The effective date of the merger
4. The non-refundable Amended Articles of Incorporation Filing Fee ($65.00) can be submitted via OPTins.org
