Parents and student athletes play a pivotal role in ensuring a student’s safety in extracurricular school sports.

DPH approved annual trainings for parents and student athletes

It is important for parents and students to learn the signs, symptoms, and dangers of sports-related head injuries and the elements of a safe return to play. Any student who participates in an extracurricular school activity and their parent or legal guardian must take the DPH-approved annual training every year. Below are two options for this training:

Required forms for parents and student athletes

Student athletes and their parents must fill out the Pre-participation Head Injury/Concussion Report Form for Extracurricular Activities form prior to the beginning of every sports season. This form should be submitted to the coach and school nurse and/or athletic director (depending on school policy) to ensure that it is safe for the student to participate in school sports.

Parents or legal guardians should fill out the Report of Head Injury Form if their student has had a suspected head injury outside of school extracurricular activities.

Any student athlete who sustains a concussion during extracurricular school activities must be given clearance to begin and complete a graduated return to play protocol. The Post Sports-Related Head Injury Medical Clearance and Authorization form must be completed by an authorized medical provider before returning to play.
