What is the Group Adult Foster Care Program?

The Group Adult Foster Care (GAFC) program provides personal care services to eligible individuals residing in a home setting. The personal care services are provided by direct care workers employed by a GAFC agency. The program helps individuals remain in their communities while receiving the assistance they need to maintain their independence.

Who is Eligible?

  • Adults (22 years or older) who require physical assistance or cueing and supervision with at least one Activity of Daily Living (ADL), such as bathing, dressing, or mobility.
  • Individuals who qualify for MassHealth Standard or CommonHealth.

Program Benefits

  • Personalized Support – Assistance with ADLs, medication reminders, nursing oversight and case management.
  • Care Coordination – A registered nurse and case manager provide oversight and personalized care planning of the GAFC personal care services.
  • Care Delivery - A direct care aide provides daily physical assistance or cueing and supervision to each GAFC member in the member’s home.
