How to request a copy of your COVID-19 vaccination record.

Massachusetts may have a record of vaccinations you received in the Commonwealth. This may include COVID-19, influenza, tetanus, and many others. The My Vax Records tool lets you access your COVID-19 digital vaccine card and your vaccination history from the Massachusetts Immunization Information System (MIIS).

Options

  • If you were vaccinated by your primary care provider, at a community health center, or local health department, contact your primary care provider.
  • If you were vaccinated at a local health department or “pop-up” clinic: contact your primary care provider.
  • If you were vaccinated at a Massachusetts mass vaccination site, you can contact your primary care provider or follow these instructions:

    Eastfield Mall January 25 - July 6: View vaccination record in unique email sent by Curative after each vaccination dose or call Curative’s Support Line at (888) 702-9042 for assistance.
    Danvers Doubletree January 25 - June 30: View vaccination record in unique email sent by Curative after each vaccination dose or call Curative’s Support Line at (888) 702-9042 for assistance.
    Former Circuit City - Dartmouth February 24 - July 30: View vaccination record in unique email sent by Curative after each vaccination dose or call Curative’s Support Line at (888) 702-9042 for assistance.
  • If you were vaccinated at one of these retail pharmacies: Stop and Shop, Hannaford, Big Y, Price Chopper, Wegmans, Rite Aid, and Costco, you may visit any store of the retail pharmacy chain where you were vaccinated and ask the pharmacist for a copy of your vaccine record. You will need to verify some personal information for security and patient privacy purposes.
  • If you were vaccinated at one of these retail pharmacies: follow the directions below. If you do not see your pharmacy listed, please contact them directly for directions for obtaining your vaccine record.

    CVS Sign into your CVS health account (you may have set this up when booking your vaccination appointment) to view vaccination records and COVID-19 test results. This is also available via the CVS Health mobile app.

    Visit any CVS store and ask the pharmacist for a copy of your vaccine record. You will need to verify some personal information for security and patient privacy purposes.

    The pharmacy staff can help you access your CVS health account to get a digital vaccine record or provide you with a paper copy of the vaccine record.

    Walgreens Sign into your Walgreens account (you may have set this up when booking your vaccination appointment) to view vaccination records and COVID-19 test results. This is also available via the Walgreens mobile app.

    Visit any Walgreens store and ask the pharmacist for a copy of your vaccine record. You will need to verify some personal information for security and patient privacy purposes.

    The pharmacy staff can help you access your Walgreens account to get a digital vaccine record or provide you with a paper copy of the vaccine record.

    Walmart Open the Digital Vaccine Record webpage on your mobile web browser or desktop computer. (Not the Walmart mobile app). Select the View record button. Sign In and enter your Walmart.com registered email and password. Continue to sign in to your pharmacy account (or add Pharmacy Access if your Walmart.com account is not yet connected to the online pharmacy). Once you are connected to the online pharmacy, select Digital Vaccine Record from the left-side menu.

    Visit any Walmart store and ask the pharmacist for a copy of your vaccine record. You will need to verify some personal information for security and patient privacy purposes.

    The pharmacy staff can help you connect your Walmart account to the online pharmacy and get a digital vaccine record or provide you with a paper copy of the vaccine record.  You can also request a medical summary that includes a vaccine record.

    Shaw’s or Star Market

    Text "Record" to (844) 590-0015 OR use the link sent to the email address used when scheduling appointment.

    A unique QR code digital vaccine record will be sent once personal information has been verified.

    Visit any Shaw’s or Star Market pharmacy and request a copy of the vaccine record (no appointment needed). You will need to verify some personal information for security and patient privacy purposes.  Shaw’s or Star Market can re-issue a CDC card, or reprint the receipts with vaccine brand and dates on it, or print form registry

If the above options are not possible, complete an Immunization Record Request Form that goes directly to the state Department of Public Health. You will receive a complete history of all vaccines you have received, not just the COVID-19 vaccine. This process may take up to 2 weeks. Please note that you will receive a paper record. You will not receive a COVID-19 vaccine card.

If you are unable to use any of the above options, or you have difficulty accessing the internet, please contact the COVID-19 Vaccine Resource Line (Monday through Friday from 8:30 a.m. to 6 p.m., Saturday and Sunday 9 a.m. to 2 p.m.) by calling 2-1-1 and following the prompts. The COVID-19 Vaccine Resource Line is available in English and Spanish and has translators available to in approximately 100 additional languages.
