Send DOR a Copy of My Court Order

How to send your court order to DOR.

If you have a court order that DOR does not yet have on file, please send a copy by email to cssgen@dor.state.ma.us or fax to 617-660-3609.   

Remember to send both a copy of the court order and any stipulation or agreement signed by the parties if you have one.
