TERT Application

Below you will find the TERT application and applicant requirements.

In order to apply for consideration to the team applicants must:

• Complete COMU Application.
• Complete COMU Home Agency Certification Form.
• Must have a signed letter from the agency director acknowledging/approving participation on department letterhead. 
• Provide proof of required certifications for desired role. 
• Email all of the above mentioned to [e-mail].
